Right now, all of our initial visits are done remotely. Please call us to schedule an appointment or a phone call with one of our case managers. If you are having an emergency, such as suicidal thoughts, thoughts of harming yourself or another, or a drug reaction, please call 911 or go to the emergency room immediate.
Many people with mental health conditions and psychosocial disabilities face poor-quality care and violations of their human rights, which demands profound changes in mental health systems and service delivery. Our mission here at Acute Care Management is to help increase a positive change in our members lives and educate our members about mental illness. We promote a person-centered, rights-based, recovery approach to mental health systems, behavioral health and services. Our goal is to effectively diagnose, assess, and treat clients with the least amount of medication possible, to ensure that people with mental health conditions are included in the community and are able to lead full and meaningful lives.. Its time to "stand on your past and look the future in the face" as stated by Clifford W. Beers in 1908, in his autobiography, A Mind That Found Itself.
Targeted Case Management is a collaborative assessment, planning, facilitation, and advocacy process for options and services to meet a person's recovery needs. The case manager coordinates services and helps oversee the Person-Centered Plan. Staff will link individuals to needed services and monitor the results. Targeted Case Management is intended for people who require several mental health services and have difficulty accessing or sustaining services independently. Each year the government spends billions to combat substance abuse so people with substance abuse or mental illness can receive the care they need without worrying about the cost. Because of this, Medicaid created targeted case management in 2007, so these patients could receive the proper care they deserve. Our experienced professionals put your healing needs first. Our case managers work closely with patients, families, providers, and payers (managed care organizations) to develop, implement and monitor personalized holistic care plans. In 1893, Isaac Ray, a founder of the American Psychiatric Association, provided a definition of the term mental hygiene as "the art of preserving the mind against all incidents and influences calculated to deteriorate its qualities, impair its energies, or derange its movements." In the post-WWII years, references to mental hygiene were gradually replaced by the term 'mental health'. In the broader context, critical social determinants that impact people's mental health such as violence, discrimination, poverty, exclusion, isolation, job insecurity, or unemployment, and lack of access to housing, social safety nets, and health services, are factors often overlooked or excluded from mental health discourse and practice. In reality, people living with mental health conditions and psychosocial disabilities often face disproportionate barriers to accessing education, employment, housing and social benefits - fundamental human rights - on the basis of their disability. As a result, significant numbers are living in poverty.
